Nurse will consult with Medical Care-A-Van staff, setup onsite appointment, provide consent forms to facility, collect completed consent forms from the facility, arrive to the facility 30 mins prior to appointment time for setup the day of the schedule appointment, and assist with getting children to and from the medical Care-A-Van safely the entire time of the visit so that the daycare facility staffing is not compromised on the day of the visit.
Medical Care-A-Van team visits schools, daycares and community sites to provide physical exams, lead & hemoglobin screening (“blood work”), hearing & vision screening, immunizations and other health screenings. The team is made-up of a provider (nurse practitioner or physician), a medical assistant and a driver.
The Care-A-Van will also service the siblings of childcare children with signed consent form, however the parent/guardian or designated person must accompany the sibling.
